Wake up with eyelids that feel glued shut, lashes crusted with tiny flakes, and eyes that burn a little by evening? That's very often blepharitis. So what is blepharitis, really? Just inflamed eyelid edges. The lash line, or the small oil glands tucked inside the lid, or both. It isn't infectious, you won't pass it to your family, and it hardly ever harms vision. What it does do is hang around. Months, sometimes years. That's why most people searching for blepharitis treatment aren't after a quick fix, they want something that lasts. The good news is that most of it can be handled at home. The not so good news is that if it's ignored, the same blocked glands can turn into a painful lump that ends up with our oculoplastic surgery team, or the surface of the eye gets irritated enough to need cornea treatment. Sorting it out early saves you all that.
Dr. Vaishal Kenia, Chairman and Medical Director at Eye Hospital in Mumbai, says: "People walk in hoping for one magic drop. I wish. Blepharitis is more like brushing your teeth, you can't do it once and be done. The patients who do best aren't the ones with the fanciest prescription, they're the ones who clean their lids every single day, even when it's boring and even when the eyes feel fine."
What Is Blepharitis and What Causes It?
Swollen, Crusty Eyelid Edges
Doctors split it in two. Anterior is the front, along the lashes. Posterior is the inner edge of the lid where the oil glands open. Lots of people have both at once, so don't worry too much about the label. What you'll notice is the burning, the grit, the red rims. Easy to mistake for dry eyes, and to be fair the two often turn up together.
Bacteria and Dandruff at the Lash Line
Everyone has bacteria on their lids. In some people they multiply, or dandruff from the scalp and eyebrows drifts down onto the lashes, and the lids get angry. You'll see flakes at the base of the lashes and sometimes the lids are stuck in the morning. A red eye alone doesn't mean blepharitis though. One bright red patch on the white is usually just a broken blood vessel.
Blocked Oil Glands and Rosacea
Along the inner lid sit small glands that make the oily layer of your tears. When they block, the oil turns thick and waxy, a bit like toothpaste, and tears dry up too fast. Eyes burn even when they're watering. Rosacea on the face runs with this a lot. Sometimes one gland swells into a hard lump, a chalazion, and if it won't go down our oculoplastic team can deal with it.
Demodex Mites
Almost every adult has tiny Demodex mites living in the lash roots, and mostly they do nothing. Too many of them, though, and you get itching that's worse at night, plus a crusty ring around the lashes like a little sleeve. People rarely guess this one. A doctor spots it under magnification in seconds, so if your lashes just won't stay clean, book an eye test.
How Is Blepharitis Treated Long Term?
Daily Lid Hygiene Is the Backbone
Warm compress first, five minutes or so, to soften the crust. Then clean along the base of the lashes with a lid wipe or whatever cleanser your doctor suggests. Every day. Yes, even on the good days. If your eyes still feel dry after a few weeks of this, ask for a dry eye evaluation.
Medicines for Flare Ups
No medicine cures it for good, so think of them as flare up tools. An antibiotic ointment for a week or two settles the bacteria, a mild steroid for a few days if the lids are very inflamed, and for the oil gland type some doctors add an oral antibiotic for a few weeks. Mites have their own treatment. Please don't use leftover steroid drops from some earlier problem, that can backfire. Book a consultation first.
In-Clinic Help and Stubborn Lumps
If warm compresses and cleaning aren't enough, the clinic can help open up blocked glands. A chalazion that's been sitting there for weeks and won't shrink may need a small procedure, which the oculoplastic surgery team does regularly. And don't squeeze it yourself.
Keep Going and Watch the Cornea
The biggest mistake is stopping the day the eyes feel better. That's how it creeps back. Take eye makeup off every night, and if you wear kajal on the inner rim, skip it during a flare up. Same for contact lenses. And if you ever get real pain, light that hurts, or blurry vision, come in, because long standing blepharitis can irritate the cornea and need cornea treatment.
